Individual Planning

People’s expectations are changing with the move away from a ‘one size fits all’, block funding model to increasingly individualised approaches and thinking. There is an increased focus on entitlement, on human rights and delivering support that strengthen what people can do rather than focus on what people cannot do.

futures Upfront is committed to working together with individuals, staff, management and boards to plan and get the most out of the changes in order to deliver better, more individualised supports and services.

Some recent work:

Following the release of funding for 300 people wanting to increase their skills and knowledge to become more independent and increase their independent living skills, futures Upfront under contract from the NSW Council for Intellectual Disability trained almost 100 people with intellectual disability and assisted them with their individual plans.

the answers with them. For the SLF project these skills combined with her engaging manner ensured that participants found the experience meaningful.”

Community Options Australia commissioned futures Upfront to develop and deliver a six week training program focusing on the skills, practice and tools needed by Care manager to ‘enable good lives’.  Following the release of a discussion paper flagging that Case Managers may no longer be funded in the new Home Support Program, many people are looking to expand their skills and knowledge to prepare for the future.

As such I feel I have the unique responsibility to stay on track with workload and responsibilities and also my own self-care.”
